Abstract

Allergic bronchopulmonary aspergillosis (ABPA) is a pulmonary disorder of asthmatic patients and patients with cystic fibrosis (CF) arising from an allergic response to antigens expressed by Aspergillus fumigatus colonizing in the bronchial mucus. The diagnosis may be delayed because many clinical and serological diagnosis criteria for ABPA overlap with common manifestations of the lung disease especially in CF. The disease can finally progress to fibrotic lung disease. Once the disease is recognized and treatment is commenced, progression to end-stage lung disease could possibly be prevented. Treatment of ABPA has two main targets. The first is modulation of inflammation and immunologic reactivity, for which systemic corticosteroids are the mainstay of therapy. The second is attenuation of the antigen burden arising from fungal colonization and successful treatment with itraconazole has been reported in numerous studies. In this article, we review the diagnosis and treatment of ABPA in light of recent investigations.
