Abstract

Kawasaki disease (KD) is a generalized systemic vasculitis predominantly involving medium-sized arteries. The Kawasaki disease shock syndrome (KDSS) was reported firstly as myocardial dysfunction, earlier onset, coronary artery involvement, and poor response to standard intravenous immunoglubulin (IVIG) therapy. A three-month-old girl was admitted with high fever, irritability, poor feeding and hemodynamic instability. For the presence of pyuria, fever and clinical sepsis suspicion, intravenous empirical antibiotic treatment was commenced with a preliminary diagnosis of urosepsis. Thereafter, an echocardiographic investigation was performed because of persistent fever and increasing platelet counts. Kawasaki disease was diagnosed by determining coronary artery dilation on echocardiography. Here, a young infant with KDSS was reported to emphasise this uncommon presentation of Kawasaki disease, because of possible fatal outcome without timely IVIG treatment.
